Home Health and Hospice Care

Home Health & Hospice Care provides home care and hospice services in 25 communities in southern New Hampshire, including Wilton. They provide traditional home care, inclusive of nursing, rehabilitation, therapies, medical social work, and licensed nursing assistants. Their hospice team cares for patients in their homes, in nursing facilities, and in the Community Hospice House, which is owned and operated by Home Health & Hospice Care.

Meals-on-Wheels

St. Joseph Community Services, Inc. provides delicious and nutritious meals at numerous dining centers throughout Hillsborough County, N.H. , and volunteers deliver Meals on Wheels and friendly visits to homebound, older (60+) and disabled adults. Clients must be at home to receive meals, meals will not be left. Even more than a meal, clients receive a hot lunch, daily wellness check and information on other social service programs.

The Monadnock Adult Care Center offers supportive and structured care during the day to adults with physical, emotional, or social challenges. Our mission is to improve the quality of life of our members, their families, and caregivers while we support an individual’s efforts to remain independent. Our program helps older adults remain in their homes as long as possible. It provides respite care for families. We offer activities that maintain or improve the physical, mental, and social well-being of our members.
Services include:
- Medication administration
- Daily health screening and group discussions, including current events
- Counseling for individuals and families
- Activities like Poetry and short-story readings, craftwork and art projects
- Strong Living exercise programs
- Community outings and service projects
- Personal care assistance and nursing assessment
- Family support groups including Alzheimer’s caregivers support
- Information and referrals to other community services
Monadnock Family Services

Just like physical health problems, mental health challenges come in many shapes and sizes. Services for adults through Monadnock Family Services aim to help people who face disabilities from their conditions. After a careful initial interview, our counselors will help you understand your options for care; we’ll determine whether our services are the best fit for your individual needs. The counselor may suggest you schedule an interview with one of our psychiatrists. If we aren’t the right provider to help you, we’ll make recommendations for the next steps you might take.
Senior Housing
Edgewater Estates is an affordable senior rental housing community located in downtown Wilton, with 1-bedroom apartment units. Income restrictions may apply, to learn more click HERE.

Souhegan Valley Rides (“The Blue Bus”)

Souhegan Valley Rides (SVR) is a "dial-a-ride" type demand response bus service available to residents of Amherst, Brookline, Hollis, Milford, Mont Vernon and Wilton NH. The focus for this service is on providing transportation to non-emergency health care appointments and assisting those in greatest need – elderly residents, those living with a disability, and residents who are unable to drive. Other residents may use the service on a space-available basis.

Wilton Property Tax Exemption for low-income seniors
Wilton provides a property tax exemption for qualifying low-income seniors. The exemption amount is deducted from the overall assessed value of the property. The property must be the applicant's principal residence, and the taxpayer must have a net income not exceeding $35,000 for a single person or $45,000 for a married couple. Adult and Aging Care

Adult and Agine Care provides a variety of social and long-term support to adults age 60 and older, and to adults between the ages of 18 and 60 who have a chronic illness or disability. A variety of social and long-term services and support can be accessed through the ServiceLink Resource Center and the NH DHHS District Offices. Services and support are intended to assist people to live as independently as possible in safety and with dignity. Examples include:
